EXERCISE: Each session is 45-50 minutes (depending on intensity) and will be scheduled for one hour. (This creates a time buffer for the client and trainer to transition in and out of the session comfortably.) The time buffer before or after a session can be used by a client to warm up or cooldown (depending on session time and the facility’s hours of operations). The client can use the warm-up area or an empty station unless specified differently by the studio owners. The training program will be based off of NASM's Optimal Performance Training (OPT) Model. The OPT Model is based on scientific evidence and principles that progresses an individual through five training phases: stabilization endurance, strength endurance, hypertrophy, maximal strength and power. General Structure of a Session: 3min: Foam Roll 3min: Core Activation 4min: Warm Up & Dynamic Stretch 30-35min: Resistance Training 5min: Cool Down & Passive Stretch GOALS/PROGRESS: Specific goals need to be presented to the trainer in a “SMART Goal” format. Coach Siepiora does not require you to have a goal. If the client does not present a specific goal, Coach Siepiora will guide the client through a cyclical training module that follows NASM’s OPT Model based on their current physical capabilities. The trainer will collect data regarding the client's strength, endurance, and mobility during sessions. Body composition, sleep monitoring, and nutrition tracking will be the responsibility of the client. If a client needs assistance to set a SMART Goal, they must communicate this via Whatsapp to request the trainer to send the SMART Goals Worksheet. The client and the trainer will then collaborate to complete the worksheet together. PAYMENT POLICIES: The client will pay the amount of $300 at the beginning of the 4 week period.. The client will receive 4 session credits at the beginning of the 4 week period. The client will receive an electronic receipt via email from the training portal. The receipt can also be found on the training portal using their desktop option. COMMUNICATION POLICIES: All matters regarding scheduling, cancellations, and invoices need to be communicated to the trainer via WhatsApp. Communication with the trainer regarding the client’s program outside of their sessions will occur via WhatsApp. Asking questions, sending recordings of yourself performing an exercise for review, or sending images of the client’s nutrition compliance is encouraged for accountability and support. It is the client’s responsibility to establish a communication style and physical boundaries with the trainer. Any verbal or electronic communication that is perceived as threatening, harmful, or inappropriate to the trainer may be considered grounds for dismissal, which will result in immediate termination of the service and no refund of the active training period. SCHEDULING & CANCELING A SESSION: Sessions with more than a 24-hour notice can be rescheduled within the 4 week period. Any session with less than a 24-hour cancellation will be forfeited. The trainer and client will find a set time for a recurring schedule. The trainer and client will assume that the session will remain at that time unless either individual needs to reschedule or cancel their session. CANCELING THE PROGRAM: There will be no refunds for canceling the 4 week training agreement. RELEASE OF LIABILITY: Because physical exercise can be strenuous and subject to risk of serious injury, your personal trainer urges you to obtain a physical examination from a doctor before beginning any exercise or training program. You agree that by participating in these physical exercise sessions or personal training activities, you do so entirely at your own risk. This includes, without limitation, (a) your use of all amenities and equipment in the facility and any off site location and your participation in any activity, class, program, personal training or instruction, (b) the sudden and unforeseen malfunctioning of any equipment (c) our instruction, training, supervision, or dietary recommendations. You agree that you are voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury. You expressly agree to release and discharge your personal trainer or instructor, and from any and all claims or causes of action. This waiver and release of liability includes, without limitation, all injuries to you which may occur, regardless of negligence. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from. You acknowledge that you have carefully read the terms and conditions and understand that it is also a release of liability. FOUNDATIONAL NUTRITION EDUCATION AND GUIDANCE: The basics of calories and macronutrients (carbohydrates, proteins, and fats) and how they influence your health. An understanding of fiber, water, and micronutrients (vitamins and minerals) and their role in overall wellness. How to interpret numerical values in your food tracking to make informed decisions. The term "guidance” is used to articulate that the nutritional advice provided should be viewed as a reference tool to create your own goals or to adhere to a program created by a licensed dietitian/nutritionist rather than a strict dietary plan you must follow. Nutrition guidance is not provided with the intent to cure any chronic illnesses or disease. In accordance with Illinois laws, it is also not intended to dictate what you must specifically eat. PROGRESS: Coach Siepiora does not require you to have a goal. Accountability through completing workouts and tracking nutrition habits is the first goal Coach Siepiora has for his clients. Sharing body composition and nutrition tracking data is encouraged to be shared with as much transparency as the client feels comfortable with. With more accurate data, the service will yield better results. If the client wants the coaching to assist in reaching a goal. They must format their goal into a S.M.A.R.T. goal. The trainer can provide more information on how to set a S.M.A.R.T. goal per the request of the client. PAYMENT POLICIES: The trainer believes in making this service accessible to everyone. Pay What You Can from $200 – $500 per month. ($350 is the Ideal Price) Regardless of payment amount, clients will receive the same service. Paying more helps the trainer offset their costs to keep their service accessible. This release includes, but is not limited to, personal injury, illness, death, and property damage, except to the extent that these were caused by the gross negligence or willful misconduct of Coach Siepiora LLC. IV. Client Responsibilities: As a client, you agree to disclose honestly your dietary, medical history, and other health-related information. You commit to following the recommendations and plans as discussed and agreed upon with your nutrition coach. You understand that failure to comply with these guidelines may diminish the possible outcomes of the program. V:. Confidentiality: All information shared by you during the course of your nutrition coaching will be kept confidential by Coach Siepiora LLC. Information will only be disclosed with your written consent or as required by law. You acknowledge that you have carefully read the terms and conditions and understand that it is also a release of liability.
