This 12 week program is for individuals who had a recent cardiac event, has a chronic heart condition, has family history of heart disease, are living with diabetes or a BMI over 30. This program includes guidance by a Certified Exercise Physiologist who provides weekly personalized exercise prescriptions that include detailed instructions and exercise videos. We also provide education materials based on your condition and regular check ins with our Licensed Nurse Practitioner. This program can be done at home or a place convenient for you through our app! *Medicare Part B participants may be eligible
This class will incorporate walking with body weight and resistance exercises to improve your heart health, strength and balance. Each class is led by a Certified Exercise Physiologist and individuals will have their heart rate and blood pressure checked before and after each class and exercise intensity is monitored throughout class. This class is designed for individuals who had a recent cardiac event, graduated from a cardiac rehab program, has a chronic health condition, is living with Type 2 diabetes or a has a BMI over 30. Classes will be held starting September 12th 2023 on Tuesdays and Thursdays at the Presbyterian Church of Western Springs, Jones Room.

This program includes guidance by a Certified Exercise Physiologist who provides weekly personalized exercise prescriptions that include detailed instructions and exercise videos. We also provide education materials based on your condition and regular check ins with our Licensed Nurse Practitioner. Phase 3 is our monthly program for individuals who completed a Cardiac Rehab Phase 2 program, our 12 week Cardiac Recovery or Cardiac PreHAB program and want to continue to improve their heart health at home or at a place convenient for them. This program includes guidance by a Certified Exercise Physiologist who provides weekly personalized exercise prescriptions that include detailed instructions and exercise videos. We also provide education materials based on your condition and regular check ins with our Licensed Nurse Practitioner. This program can be done at a place convenient for you through our app! *Medicare Part B participants may be eligible
